It is described as a root cause of autism, a driver of PANS/PANDAS flares, the explanation for food sensitivities, the reason behavioral symptoms fluctuate, and the problem that a specific supplement or protocol will solve. Some of this is grounded in legitimate science. Some of it is not. And because parents in this community have often been burned by promises that outrun evidence, you deserve a clear-eyed look at what leaky gut actually is, what the research actually shows, and what is worth doing about it.

What Intestinal Permeability Actually Is

The lining of the small intestine is one cell layer thick. Those cells are held together by structures called tight junctions — essentially the seals between cells that control what passes through the intestinal wall and into the bloodstream. In a healthy gut, tight junctions allow nutrients to pass through while keeping bacteria, undigested food particles, and inflammatory compounds inside the gut where they belong.

When tight junctions are disrupted — by inflammation, infection, certain medications including NSAIDs and antibiotics, an imbalanced microbiome, or a diet low in fiber — the barrier becomes more permeable. Substances that should stay in the gut begin to pass into the bloodstream. The immune system, encountering these unfamiliar compounds, mounts a response. That immune response produces systemic inflammation.

Where the Marketing Goes Beyond the Science

The significant overstep in the leaky gut conversation is the claim that sealing the gut lining with supplements will resolve behavioral symptoms in autism or PANS/PANDAS. The evidence does not support that claim — not because intestinal permeability is not real, but because:

  • The relationship between gut permeability and behavior is associational, not established as directly causal for behavioral symptoms

  • Most supplements marketed as "gut sealers" have limited rigorous evidence for their effect on tight junction function in humans

  • Addressing gut permeability without addressing the underlying drivers — inflammation, dysbiosis, dietary factors — is unlikely to produce lasting change

  • No supplement can substitute for a comprehensive clinical evaluation of gut health
